1:1 School Based - Lpn Licensed Practical Nurse Carbondale, PA area Job Details for the Lpn Licensed Practical Nurse:

Local School District; Life Skills Classroom
Up to $29 per hour
Monday through Friday, hours between 7 am and 4 pm
ride transportation to/from school

Essential Functions for the Lpn Licensed Practical Nurse:

Participates in Ongoing Interdisciplinary Assessment of the Patient.
Participates in the Development and Evaluation of the Plan of Care in Partnership with the Patient, Representative (If Any), and Caregiver(S).
Recognizes and Reports Changes in the Patient’s Condition to the Rn Supervisor.
Counsels and Educates the Patient and Family in Meeting Healthcare Related Needs.
Completes Timely and Accurate Clinical Notes Including Addressing Patient’s Progress.
Assists the Rn in Carrying out the Patient Plan of Care.
Assists the Client in Learning Appropriate Self-care Techniques.
Follows Accepted Standards of Nursing Practice.
Participates in In-services or Training, As Requested.
Participates in the Office’s Emergency Preparedness Program

Minimum Requirements for the Lpn Licensed Practical Nurse:

Current License As a Lpn/Lvn in the State(S) in Which He/She Practices
Minimum One (1) Year Relative Experience; Required
Cpr Certification
Valid Driver's License
Willing to Undergo Pre-employment Screenings
