As a Physician, you will be responsible for coordination of proactive care for patients with chronic conditions as well as identifying wellness and/or preventative service for clinical patients
Completed Residency Program
Board Certification in Family Medicine
Indiana State Medical License, DEA, and CPR certifications
Ongoing Continuing Medical Education (CME)
1-3 years of experience as a physician preferred (but not required)
